Paper forms are a liability

Construction sites generate dozens of forms every day. Toolbox talk records, daily site inspections, permits to work, near miss reports, plant pre-inspections. On most sites, these are paper forms filled in with a pen, filed in a site office, and forgotten until someone asks for them during an audit.

Paper forms get lost. They get wet. Handwriting is illegible. There is no reliable way to prove when a form was completed, who completed it, or whether it was reviewed by the right person. When the HSE requests your daily inspection records for the last six months, retrieving them from a filing cabinet is not a quick exercise.

CDM 2015 places specific duties on principal contractors to plan, manage, and monitor site safety. Permits to work, method statement sign-offs, and inspection records are key evidence that these duties are being discharged. Losing or misfiling a single form can undermine months of compliance effort.

How AttendIQ replaces paper forms

AttendIQ gives you a complete digital forms system with standard construction templates, a drag-and-drop builder for custom forms, and mobile submission with automatic PDF archiving.

15 standard templates

Every new tenancy is seeded with 15 form templates covering daily inspections, toolbox talks, permits to work (hot works, confined space, working at height), COSHH assessments, method statement sign-offs, first aid reports, near misses, plant pre-inspections, accident reports, visitor logs, delivery receipts, and return to work declarations.

Custom form builder

Build your own forms using a drag-and-drop interface. Add text fields, dropdowns, checkboxes, date pickers, photo capture, GPS location tags, and digital signature fields. Scope forms to specific sites or make them available organisation-wide.

Mobile submission

Workers and supervisors complete forms directly from the AttendIQ app on site. Capture photos, tag the GPS location, and sign with a finger. Every submission is timestamped and linked to the worker, site, and zone automatically.

Approval workflows and PDF archiving

Configure forms to require approval before they are finalised. Approvers receive a notification and can approve, reject, or request changes. Every completed form is automatically archived as a PDF in your document management system, searchable and audit-ready.

How it works

1

Choose or build

Use one of 15 standard templates or create your own with the form builder. Assign forms to sites and set who can submit (supervisors only, or all workers).

2

Submit on site

Workers and supervisors open the form in the app, fill it in with photos, location, and signatures, and submit. The form is available in the admin portal immediately.

3

Review and archive

Managers review submissions, approve or reject, and the final form is archived as a searchable PDF. All records are retained for the applicable compliance period.

Frequently asked questions

What standard form templates does AttendIQ include?

AttendIQ includes 15 standard form templates designed for UK construction: daily site inspection, toolbox talk record, permit to work (hot works, confined space, working at height), COSHH assessment, method statement sign-off, first aid report, environmental incident report, near miss report, plant pre-inspection, accident/incident report, visitor sign-in log, delivery receipt, and return to work declaration. Standard templates cannot be deleted but can be archived if not needed.

Can I create custom forms for my sites?

Yes. The form builder lets you create custom forms with text fields, dropdowns, checkboxes, date pickers, photo capture, signatures, and more. Custom forms sit alongside the standard templates and can be scoped to specific sites or made available across your entire organisation.

Can workers submit forms from their phone on site?

Yes. Workers and supervisors submit forms directly from the AttendIQ mobile app. Forms can include photo capture taken on site, GPS location tagging, and digital signatures. Submissions are timestamped and linked to the worker and site automatically.

How are completed forms stored and retrieved?

Every completed form is stored digitally and automatically archived as a PDF in the Forms Archive folder within your document management system. Forms are searchable by type, site, date, and submitter. All form data is retained for the applicable CDM 2015 retention period and included in your compliance audit trail.

Do forms support approval workflows?

Yes. You can configure forms to require approval from a site manager or other designated role before they are finalised. The approver receives a notification when a form is submitted and can approve, reject, or request amendments. The full approval history is stored against the form record.

Construction site inspection forms app

Daily site inspection records are one of the most frequently requested documents during HSE visits and insurance claims. In most cases, they are also the hardest to produce - buried in a filing cabinet or written on a wet pad that nobody can read. AttendIQ's daily site inspection template is completed on the app, GPS-tagged, time-stamped, and exported as a dated PDF the moment it is submitted.

You can filter completed inspection records by date range, site, or the person who submitted them. If an RIDDOR report references a hazard that should have been identified in an inspection, you can pull the relevant form in under a minute rather than searching through a physical archive.

Permit to work software for UK construction

Permits to work for hot works, confined space entry, and working at height are high-risk documents. A permit that is not properly completed, countersigned, or closed out is a legal and safety liability. AttendIQ includes permit templates for all three categories, with mandatory fields, required signatures, and automatic closure prompts.

Permits are scoped to the relevant site and must be approved by a named supervisor before work begins. When the work is complete, the permit is closed by the supervisor with a final sign-off. Every version, every signature, and every timestamp is stored permanently. You always know who authorised what and when.

Paperless construction forms for small contractors

Moving from paper forms to digital does not have to be a six-month IT project. AttendIQ's 15 standard form templates cover the documents most used on UK construction sites. Small contractors with one or two sites can be fully paperless within a day of signing up. Workers submit forms from the app, managers approve from the admin portal, and everything is archived automatically.

If your sites have specific forms that are not in the standard library, the custom form builder adds new templates without any code or IT involvement. Drag and drop the fields you need, add any conditional logic, and publish. The form is live on the app immediately.

Every form. Every site. Always findable.
